Is SmartCare insurance?No. SmartCare is a pre-paid package of medical care and services offered at a reduced price. It is designed for people who are ineligible for, or unable to obtain insurance because of their diabetes.
Does SmartCare include medicines or testing supplies?No. Every diabetic is different and treatments vary widely. There is no way to meet all the individual variations with a single program. We will tailor your treatment options to fit your budget whenever possible.
Will my insurance cover SmartCare?SmartCare is designed for diabetics without insurance. After every visit we will give you a receipt for the actual value of services provided. You may opt to use the receipts towards meeting your deductible for the year. We, however, will not be responsible for filing any claims related to the program. If your insurance covers your doctor's visits, we will be happy to see you as a regular patient.
What if I move away from the area?We understand that your plans may change over the course of the year. We may be able to adjust the balance due if you leave the area prematurely.
What is not included?
SmartCare does not include medicines, testing supplies, visits or lab work with any doctors or hospitals other than The Diabetes Center, or cover any services not specifically listed in the contract. You are allotted 7 visits and extensive lab work for the year. This is normally more than enough to cover your diabetic medical needs for one year.
I'm not diabetic. Can I still sign up for SmartCare?SmartCare is specifically designed for diabetics. In most cases, non-diabetics would find that paying for individual visits would be a cheaper option.
Do I have to sign a one-year contract?Yes. Managing diabetes requires a commitment to continually monitor and revise treatments and lifestyle choices. If you wish to be compliant, you should plan on a minimum of four visits per year plus lab work. In exceptional cases, we will adjust the terms of the contract to accommodate you if you are expecting to be covered by insurance in the near future.
Why is there a payment of $297 due on the first visit?If you opt for the $99 a month plan, there is a $297 payment due on the date of your first visit. This is to cover our cost for your first visit and lab work.
What is CareCredit?CareCredit is a division of GE Credit that offers zero-interest, zero-cost financing to patients. The plan offers an easy way of financing health care services for those who are not in a position to pay up front.
What if I don't use all of the benefits included?SmartCare saves you money even when you use a portion of the allotted benefits. The flexibility to return for followups, unanticipated problems or receive extra lab work at no additional cost is included to make the program comprehensive. For these reasons, we cannot refund the value of any unused benefits at the end of the year.
